GENERAL NATURE OF
POSITION:Provides outreach,

GENERAL NATURE OF
POSITION:Provides outreach,

clients and their families that are receiving services at community older adult
social service and health care programs and participating in the Golden Gateway
OA treatment programs. Provides awareness orientation to providers about family
and peer needs with respect to wellness, stigma reduction and general health
access to services in the community.
Responsible
for leading the integration, development and implementation of daily program
and services, operational contract management, coordination leadership of
substance abuse and mental health services for dual diagnosis clients.Responsible for coordinating with. other
Catholic Charities Behavioral Health Division managers on behalf of the agency
and the clients... Responsible for the collaborative leadership with the staff
of Asian Americans for Community Involvement, Inc. and overall contract
management to integrate assessment, counseling, crisis intervention and case management
support services for clients with co-existing psychiatric and chemical
dependency disorders. Provide direct services – assessment, individual
counseling, group treatment, and case management consultation.
Responsible
to implement supportive housing services at CHDC sites. Provides case
management services at housing sites. Collaborates with CHDC staff on behalf of
the residents of the respective facilities. Schedules and supervises on-site

GENERAL

Vento and other supportive housing residents to access medical, educational,
social, prevocational, vocational, rehabilitative or other needed community
services for eligible individuals. Contacts and collaborates with specific
community agency personnel who provide other case management or treatment
services for the residents of housing resources.Advocates for residents when their tenancy is
in jeopardy. Provides housing services information to residents of the
facility. Searches for qualified housing for tenants, inspects housing and
arranges for required approvals for use of the property, assists tenants in addressing
rental eligibility and in acquiring furnishing and utilities, etc. This is a temporary, 20 hours per week

needs of safety/stability, wellness, and self-sufficiency, the Family
Specialist supports parents in reducing stress and problems that might lead to
child abuse or neglect. The Family Specialist works with the Parenting Workshop
Coordinator to co-facilitate parenting classes and to provide case management
and home visitation services to eligible Vietnamese families.
The Parenting
Workshop Supervisor facilitates Catholic Charities’ Successful Parents Program
(SPP) and conducts parenting workshops for underserved Vietnamese families with
school-aged children residing in the Franklin-McKinleySchool District
attendance area to increase parents’ knowledge and practice of positive

CORALAfter-School Instructor
General Function:
Under the direct supervision of a Site Manager, the CORALAfter-School
Instructor (CASI) will be responsible for the supervision and safety of
approximately 20 school age children.Within a small group setting the After School Instructor will help
develop and carry out lesson plans, and work to raise academic achievement within
a balanced literacy model.
The goal of CORAL San José is to improve academic achievement

The Program

family centered services for the First 5 Program, for the Successful Parents
