GENERAL NATURE OF POSITION: Provides outreach, engagement, support, role modeling, coaching and/or aftercare for children, adults and/or older adult clients and their families that are participating in our mental health programs. The focus is on assisting the individual to grow and manage their recovery and their families to understand and support their recovery. This is a part time job. (12 hours per week)

GENERAL NATURE OF POSITION: Provides outreach, engagement, support, role modeling, coaching and/or aftercare for older adult clients and their families that are receiving services at community older adult social service and health care programs and participating in the Golden Gateway OA treatment programs. Provides awareness orientation to providers about family and peer needs with respect to wellness, stigma reduction and general health access to services in the community.

GENERAL NATURE OF POSITION:

Responsible for leading the integration, development and implementation of daily program and services, operational contract management, coordination leadership of substance abuse and mental health services for dual diagnosis clients. Responsible for coordinating with. other Catholic Charities Behavioral Health Division managers on behalf of the agency and the clients... Responsible for the collaborative leadership with the staff of Asian Americans for Community Involvement, Inc. and overall contract management to integrate assessment, counseling, crisis intervention and case management support services for clients with co-existing psychiatric and chemical dependency disorders. Provide direct services – assessment, individual counseling, group treatment, and case management consultation.

GENERAL NATURE OF POSITION:

Responsible to implement supportive housing services at CHDC sites. Provides case management services at housing sites. Collaborates with CHDC staff on behalf of the residents of the respective facilities. Schedules and supervises on-site case managers. Participates in BHD management and staff meetings. Collaborates with CCSJ Housing Director. Attends CHDC managers meetings.

GENERAL NATURE OF POSITION: Provides case management/brokerage services with McKinney Vento and other supportive housing residents to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible individuals. Contacts and collaborates with specific community agency personnel who provide other case management or treatment services for the residents of housing resources. Advocates for residents when their tenancy is in jeopardy. Provides housing services information to residents of the facility. Searches for qualified housing for tenants, inspects housing and arranges for required approvals for use of the property, assists tenants in addressing rental eligibility and in acquiring furnishing and utilities, etc. This is a temporary, 20 hours per week position.

GENERAL NATURE OF POSITION: Provides outreach, education and training and mental health rehabilitation services and treatment counseling services to assist older Latino, Vietnamese, Chinese and Filipino adults whose symptoms have identified them for diagnosis and treatment. Participates as a team member to perform outreach, community provider training and education and coaching clients in daily living tasks.

GENERAL NATURE OF POSITION: Provides case management/brokerage for older adults to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible individuals. Provide basic counseling and crisis intervention services for individuals as needed. Works with family partners to assist the clients in their recovery and management of chronic mental illness, possible substance use and chronic health disorders... In the spirit of the program, “do whatever it takes” to assists the client in accomplishing his/her goals. This is a part time position.

POSITION SUMMARY:  Provide direct psychiatric and medication support services to older adults in a team environment. Makes home visit assessments as planned or in crisis situations. Must be available for on-call coverage for medication support and/or crisis situations.  The number of hours for service is established with each individual.  Hourly rate to be established based on experience.  This is a part time, 6 hour per week position.

GENERAL NATURE OF POSITION: Provides supervision and training of other therapists and mental health counseling and substance abuse treatment for clients and their families. According to special assignments may provide service site management and coordination with collaborative partners. This is a temporary part time position. (20 hours per week).

POSITION SUMMARY:

The Family Specialist works with underserved Vietnamese parents with school-aged children living in the Franklin-McKinley School District attendance area who are enrolled in the Successful Parents Program (SPP) to increase parents’ knowledge and practice of positive parenting behaviors and to improve family functioning. By addressing core family needs of safety/stability, wellness, and self-sufficiency, the Family Specialist supports parents in reducing stress and problems that might lead to child abuse or neglect. The Family Specialist works with the Parenting Workshop Coordinator to co-facilitate parenting classes and to provide case management and home visitation services to eligible Vietnamese families.

Description of Position:

An interpreter/translator is needed to help facilitate communication between FIRST 5 clients and staff who do not speak the clients’ native languages(s). The interpreter/translator will provide interpretation services during meetings between staff and clients and may also be called upon to prepare written translations of relevant documents. The individual hired must be ‘on-call’ and available to provide interpretation and translation services when the need arises.

POSITION SUMMARY:

The Family Care Manager works with under-served families with children 0-5 years. The Care Manager works as part of a culturally responsive team collaborating with key community stakeholders such as school districts and community based organizations to coordinate and secure a variety of services for families with children aged 0-5 to support their healthy development and successful entry into school. Collaboration will also include early child development experts such as mental health clinicians. Family Care Managers assist all family members to identify their own self-sufficiency barriers and work closely with them to secure appropriate support services to meet their unique needs.
